Dues Schedule
Annually, OFDA members are mailed dues invoices in November. Deadline for remittance is February 28th.
Dues are calculated by a base fee, plus the number of death certificates filed for all locations of the firm. The price per death certificate is categorized in tiers below. Death certificate amounts go through each applicable tier at that rate.
EXAMPLE: 650 death certificates = (500 x $3.50) + (150 x $1.75) =$2,012.50 + $220 base fee =$2,232.50 due.
Base fee of $220 (one base fee per company, not per branch location)

1-500 @ $3.50 per death certificate
501-1,000 @ $1.75 per death certificate
1,001-1,500 (max) @ $1.00 per death certificate

Eligibility
OFDA Bylaws - Article III
A. Firm Members
Funeral homes duly licensed under the laws of the State of Ohio that pay dues in accordance with the dues schedule approved by the Board of Directors are eligible to submit a written application to the Association for membership as a Firm Member. The Executive Committee, after receiving the report of Executive Director on such application, may approve or reject it at any regular or special meeting of the Executive Committee. Each Firm Member shall be entitled to one vote, which vote shall be cast by a duly authorized officer or other duly authorized representative of the Firm Member.
All funeral directors and embalmers who are duly licensed under the laws of the State of Ohio and who own or are employed by a Firm Member shall be known as Affiliates and shall be entitled to receive those benefits which the Association makes available to such Affiliates. In order to be regarded as employed by a Firm Member, the Affiliate’s license must be displayed with the Firm Member as required by Section 4717-7-02(I) of the Ohio Administrative Code. Affiliates may serve as Directors, officers or members of any committee of the Association unless otherwise provided in these Bylaws.
B. Branch Members
Funeral homes duly licensed under laws of the State of Ohio that are under common ownership with the Firm Members recognized as members under Paragraph A of this Article III and that pay dues in accordance with the dues schedule approved by the Board of Directors are to submit a written application for membership as Branch Members. The Executive Committee, after reviewing the report of the Executive Director on such application, may approve it or reject it at any regular or special meeting of the Executive Committee. Branch Members recognized under this membership class shall not be entitled to vote.
